Please find below the schedule of days to turn in your work hours/journal.  This is mandatory to be turned in, on time to me, at the Career Center by the end of each designated school day.  Be sure this form is signed by your employer and the hours are totaled.  When you turn in this form, an additional form (s) will be given to you.

Failure to turn in your work hours on time will result in 5 points taken off your grade, for each school day this is late.  It is mandatory that these hours are to be documented for the student to receive credit; there for if no hours are turned in, the student will receive a grade of “F”.

·        The STUDENT Evaluation forms will be due (Supervisor Completes):

          FIRST SEMESTER – OCTOBER 2 AND DECEMBER 11

          SECOND SEMESTER -   MARCH 4 AND MAY 13

·        The COURSE Evaluation form will be due (Student Completes):

          FIRST SEMESTER – DECEMBER 11 (1st Semester Students Only)

SECOND SEMESTER – May 13 (New 2nd Semester Students Only)
